רבון העולמים אלקי האלקים ואדוני האדונים המאציל הבורא יוצר ועושה. מלך מלכי המלכים. מוכתר בכתר מלכות המתלבש בעשר ספירות בלימ"ה. מגיד מראשית אחרית. אשר ברא ויצר את האדם להטיב עמו ברוב חסדיו. ולא יחפוץ במות המת כי אם בשובו מדרכיו וחי. וימינו פשוטה לקבל שבים. הנה אנכי בא אליך בעל הרחמים והסליחות. כי ידע עבדך כי אני חטאתי בתורתך ובגדתי באחדותך. ונתתי מום בקדשי שמים. ופגמתי במדות העליונות. וקלקלתי צנורי הקדושה ומנעתי להטיב משפע הראוי לבא אליהם והרחבתי גבול מלך הצפון. ושלחתי רג"ל השו"ר והחמו"ר לבער שדה תפוחים. ואדרכם באפי וארמסם בחמתי. וגם ענוש לצדיק יסוד עולם בהשחיתי את זרעי ארצה. ואנכי בראתי משחית לחבל. שורש פורה ראש ולענה. וראשו מגיע השמימה. צדיק ורע לו. כי יפול הנופל שפע אבא ואמא לנצח הוד יסוד דזעיר אנפין לאחור ולא לפנים. ופגמתי בשמותיך הקדושים. יו"ד ה"י וי"ו ה"י אל"ף ה"י יו"ד ה"י. וגם לא השבתי ידי מבלע השמות הקודש אשר באחוריהן. וקצר בה כו' כמו שנכתב לעיל אצל ואני תפלתי וסי' זה נרשם אצלו *) עד נפשי כו':

במה אקדם פני ד' אלקים ומה רפואה אבקש. ואביט ואין עוזר ואשתומם ואין סומך. כי אחרי שובי נחמתי כו'. כי נשאתי חרפת נעורי. אמרתי אפלה ביד ה' ואבוא הלום לבקש ממך מחילה סליחה וכפרה. שמתי פני כחלמיש ואדע כי לא אבוש. ועל זכות ספר התורה הקדוש אשר בידי בטחתי. ה' אמרתי אלקי אתה. כי עמך מקור חיים באורך נראה אור. או"ר זרו"ע לצדי"ק ולישר"י ל"ב שמח"ה. אנא בכח גדולת. בשם קר"ע החתום בסופי תיבות אלו. קרע ממלכת הרשעה כולה רעת האדם בארץ. וכל הטיפות שיצאו ממני לבטלה בין באונס בין ברצון בין בשוגג בין במזיד שלא במקום מצוה. בכח קר"ע יעשה עק"ר לעקור כל ניצוצות הקדושה אשר בתוך עמקי הקליפות. חי"ל בל"ע ו"יקיאנו ומבטן יורישנו אל. והניצוצות המה יעשו ער"ק לברוח מתוך החפירה ולהעלות עד צדיק יסוד עולם. כי הוא רק"ע. ושמה יקבל ש"ע אורות מהדרת פנים העליונים והנפש ההיא היא מוצאת מכל דבר רע ותעשה טוב. (הג"ה ר"ת ב' שמות אלו עולים במספר ס"ת פסוק או"ר זרו"ע לצדי"ק ולישר"י ל"ב שמח"ה. וטוב לכוין בו דהרי גוף וברית חשבונן חד) ובכן יעלה ויבא ויגיע ויראה וירצה זכות ס"ת זה לתקן את אשר פגמנו. וכאילו התעניתי ימים מספר לצדיק להעלות שמות פנים ואחור של או"א למקומ' הראשון. ואף גם זאת יתוקנו ע"י כח סגולת זכות ספר תורה הזה מה שפגמנו על דבר שבועת אלקי"ם בממ"ה שבעה הבלים היוצאים מפיו. להקיף שבעה ספירות תחתונות דמלכות. ומנעתי אותם המקיפים ממנה וגרמנו להלבישנו בהבלים טמאים רעות רוח. ואשר נדרתי ולא שלמתי ופגמתי בחיי המלך הנמשכים לו מאימא והוא מאיר בהם אל נוקבא דילי'. ומנעתי אותם מלהוריד אותם אליו ואליה. עתה על ידי חיבוק ונישוק ספר תורה זה. והבל הוידוי שאני מתודה לפניך. חטאתי עויתי פשעתי ופגמתי קדשי שמים. הנה ספר העליון ישוב כבראשונה איש את פני רעהו בחיבוק ונישוק והבל והיא כפורחת עלתה נצה. וככלה תעדה כליה. ותלבש בגדי מלכות. והרשעה כולה כעשן תכלה כי תעביר ממשלת זדון מן הארץ. אמן כן יהי רצון. יהיו לרצון אמרי פי והגיון לבי לפניך ה' צורי וגואלי: ת"ו"ש'ל"ב"ע:
