על פי ההשקפה היהודית יחסי אישות הם מצווה עצמאית, והם חשובים וחיוניים מצד עצמם גם ללא קשר לפוריות. ההגדרה ההלכתית לכך היא מצוות עונה שמחייבת יחסי אישות גם במצב שבו אי אפשר להרות כתוצאה מהם, כגון אצל אישה עקרה, זקנה, מניקה, או אישה הרה¹.

קיום יחסי אישות כשאחד מבני הזוג חולה עשוי לקבל משמעות עמוקה אף יותר, כיוון שקִרבה אינטימית כזאת מסוגלת לתת לאדם החולה תחושות של אהבה, קִרבה ותמיכה, שהן חיוניות לכל אדם, ועוד יותר לאדם החולה. תמיכה ועידוד עשויים לחזק את האדם, לשפר את הרגשתו, וממילא גם לתת לו כוחות להתגבר על המחלה. יחסי אישות מעניקים רגשות חיוביים גם לבריא מבני הזוג, וממילא מתעצמת היכולת לתמוך בחולה. היחסים הבין זוגיים יכולים להישמר במתכונתם החיובית גם אל נוכח הקשיים הגדולים שהמחלה גורמת, ומתוך כל המכלול החיובי של קִרבה, עידוד ותמיכה עשויה לצמוח ברכה גדולה.

ברור לפי זה שמותר ומצווה לקיים יחסי אישות גם בזמן שאסור להרות וגם בזמן שיש קושי לקיים יחסי אישות, הן מפאת הצדדים הפיסיולוגיים של המחלה והטיפול בה, והן בגלל הצדדים הנפשיים של חוסר רצון או בושה.

על פי תפיסת ההלכה, יש להתחשב באדם שמוגדר כאנוס, כלומר שאין באפשרותו לבצע פעולות מסוימות, ואין לדון אותו כפי שדנים אדם שנמצא במצב רגיל. הביטוי שחז"ל השתמשו בו הוא 'אונס – רחמנא פטריה', כלומר, במצבים של אונס, כשאי אפשר לקיים מצווה מסוימת, האדם לא נתבע על כך, וריבונו של עולם פוטר אותו מהתחייבות.

דוגמה לכך אפשר להביא מקיום יחסי אישות בפרט, ואינטימיות זוגית בכלל, שהם חיוניים לכל אדם הן מהצד הגופני והן מהצד הנפשי. לעיתים, עקב המחלה, נמנעת מבני הזוג האפשרות לקיים יחסי אישות במשך זמן מה, או שהם יכולים לקיים יחסי אישות חלקיים או שונים בהשוואה לעבר. מבחינת היהדות אל לו לאדם להתייסר או להצטער על מה שהוא אינו מסוגל לקיים, כי כאמור לעיל, 'אונס – רחמנא פטריה'. עם זאת, יש מקום לנסות ולקיים את מה שאפשר במצב הנתון, מתוך שאיפה להסתגל למצב החדש באופן המיטבי.

ב. במקרים שבהם אין שום אפשרות לבִּיאה באופן שתואר בהלכה הקודמת, ישאלו בני הזוג שאלת חכם האם עקב הנסיבות החריגות יש מקום להתיר להם חיי אישות 'שלא כדרכה', דהיינו ביאה בפי הטבעת⁴, ואם אי אפשר גם באופן כזה, ישאלו שאלת חכם על 'ביאה דרך איברים', דהיינו קִרבה אינטימית ללא חדירה כלל, אף על פי שהיא עלולה לגרום להוצאת זרע⁵.

היצרות הנרתיק עלולה להתרחש אחרי חלק מהטיפולים כנגד מחלת הסרטן, דוגמת הקרנות לאזור האגן, אחרי חלק מהניתוחים הגינקולוגיים, ולעיתים כחלק מתופעת ה'שתל נגד המאחסן' (GVHD – Graft Versus Host Disease) לאחר השתלות מֵח עצם מתורם⁷. אפשר למנוע או להפחית תופעה זו בעזרת אביזרים שמיועדים למניעת היצרות הנרתיק ושמירה על המצב ההתחלתי. אביזרים אלה נקראים 'מרחיבים נרתיקיים'. התייחסות לשימוש במרחיבים נרתיקיים הובאה לעיל בכרך השני, פרק כח – 'קשיים בחיי האישות אצל האישה'', עמ' 88.

⁷ מחלת השתל נגד המאחסן הינה מחלה שמתפתחת לאחר השתלת מח עצם מתורם. השתלת תאי מח עצם מתורם נעשית כטיפול בסוגים שונים של סרטן, כגון לוקמיה, מיאלומה ולימפומה. המינון הגבוה של הטיפול הכימי שניתן למחלות כאלה משמיד את תאי מח העצם החולים. תאי מח העצם מהתורם מהווים מקור ליצירת תאים בריאים. מלבד יצירת תאי מח עצם בריאים, יש לתאי השתל תפקיד חשוב נוסף והוא להתקיף את תאי הסרטן ולהילחם בהם, במידה שנשארו שרידים מזעריים של תאי מחלה לאחר קבלת הכימותרפיה. אפקט זה קשור למעשה לפעילות המערכת החיסונית של תאי מח העצם של התורם כנגד תאי הסרטן של החולה. פעילות זאת חיובית ורצויה והיא מקטינה את הסיכון להישנות המחלה. אולם לצד פעילות חיובית זו, התאים מהתורם, שנלחמים בתאי הסרטן, עלולים לפגוע ברקמות נוספות של המטופל. פגיעה זו נקראת 'מחלת השתל כנגד המאחסן'. המחלה יכולה להופיע בדרגה קלה או קשה. ביטויי המחלה יכולים להיות: שלשול, פריחה, נזק לכבד ולעיתים אף סכנת חיים. המחלה יכולה להופיע סמוך להשתלה או מאוחר יותר – חודשים לאחר ביצוע ההשתלה. הביטויים הכרוניים יכולים להיות בעור, בריריות הפה, בעיניים, בנרתיק ואף בפגיעה בכבד. עקב יובש נרתיקי, הצטלקות המקום וחוסר גמישות, עלולים להיות כאבים בזמן קיום חיי אישות, ויכולה להיווצר היצרות נרתיקית עד כדי הידבקויות בנרתיק.

כאשר יש צרכים שונים ביחסים אינטימיים בכלל ובחיי אישות בפרט, חשוב שבני הזוג ישתפו זה את זה ברצונות ובחששות. חשוב להסביר ולשתף את בן הזוג ברגשות, וכך הוא לא ירגיש דחוי⁸. אם האדם החולה מתקשה לבטא את רגשותיו הוא יכול להיעזר באנשי מקצוע. ברור שאפשר ומותר לקיים יחסי אישות גם אם רק אחד מבני הזוג מעוניין בכך, מתוך הסכמה נפשית מלאה של בן/בת הזוג. ראוי שבן הזוג או בת הזוג יעשו את המאמץ לשם כך, כמובן, בתדירות סבירה⁹.

נויטרופילים (Neutrophils) הם תאים ששייכים למערכת החיסון, ותפקידם העיקרי הוא בליעה וחיסול חיידקים או פטריות ומניעת הזיהום שהם גורמים. טיפול כימותרפי מסיבי עשוי להוריד באופן זמני את רמת הנויטרופילים בדם. כאשר רמתם יורדת מתחת ל-500 נוצר סיכון חמור לזיהום. במצב כזה אסור לקיים יחסי אישות עקב החשש לזיהומים, וגם טבילה אינה מומלצת מאותה סיבה. אומנם בדרך כלל כאשר מידת החשש גבוהה במיוחד מקובל לאשפז את החולה בבידוד כדי להגן עליו, ולרוב ההגבלות הללו לא אמורות להימשך יותר מאשר ימים אחדים לאחר שהוא משתחרר לביתו.

ג. בימים שבהם אחד מבני הזוג מקבל טיפול כימותרפי, וכן יומיים-שלושה לאחר מכן, יש להימנע מקיום יחסי אישות. גם אם הרופאים מאפשרים את קיום יחסי האישות באמצעות שימוש בכובעון (קונדום), יש להימנע מכך כיוון שמדובר בפרק זמן קצר יחסית שלא מצדיק שימוש באמצעי כזה¹².

השינויים בתפקודו של האדם החולה בבית עלולים להשפיע גם על תפקודו במערכת הזוגית והאינטימית. קשיים כאלה עלולים לאתגר את מצב הקשר הזוגי ואת מערכת היחסים הזוגית. זוגות רבים אינם מדברים הרבה על חייהם האינטימיים, אולם לאחר הטיפול במחלת הסרטן ייתכן ששגרת החיים האינטימיים תשתנה, ותאלץ את בני הזוג לקיים תקשורת פתוחה וברורה. כל זוג יחליט יחד באיזו תדירות ובאיזה אופן הם ייפגשו במקום שקט ופרטי לשיחה אינטימית, שבה יתייחסו לרגשות ולחוויות שהם עוברים – יחד, וכל אחד לחוד. בפגישה כזאת מומלץ להימנע מלדון רק בפרטים הטכניים והמנהליים. זוגות רבים מתקשים לשוחח על רגשותיהם, ודווקא זמן מחלה, שבו שגרת החיים הזוגית מופרת, הוא הזדמנות לפתח ולהעמיק את הקשר הזוגי.
